  1. Full Name
  2. Address
  3. Date of Birth
  4. Name of Parent/Guardian
  5. GPA (unweighted)
  6. Name of the college, university, community college, trade or technical school you will attend or branch of the military in which you have enlisted:

  8. List your service to the community both internal and external to your school. Briefly explain your role and reason for choosing this specific service to others.
  9. Personal Video: The scholarship committee would like to get to know you. Your 1-2 minute video will inspire us to “buy” you because you are the main star, the “product” of your 1-2 minute commercial. Remember, your video is akin to a job interview and not a “You’ve Got Talent” audition. Impress us with your professionalism and aplomb. And, PLEASE, do not simply repeat everything you already told us in the written portion of your application.
  10. The Essay: Examine the images and quotes below. Use the images and quotes to write a 300-500 word essay that tells the selection committee who you are and what you will offer the world that will inspire them to keep on learning, keep on growing. Tell us who you are and why you are the best representative of the Vitacca Mitchell Scholarship.
